📋 Insurance & Payment Information

1. What type of insurance do you accept for the 2025 year?

  • We accept out-of-network benefits for most insurance plans
  • We do not participate with any insurance plans at this time
  • This approach allows us to provide optimal care without insurance dictating your treatment plan

2. How do insurance claims work at your practice?

  • We accept out-of-network benefits for all dental plans
  • We do not file claims unless specifically requested (as a courtesy)
  • ⚠️ PAYMENT IS REQUIRED IN ADVANCE FOR ALL INSURANCE CARRIERS

14. What if I don't have insurance? Do you have a payment plan?

✅ Yes! We offer the MOTIVATIONAL DENTISTRY SMILE CLUB:

    • Child Complete: $32/month or $384/year

Adult Complete: $38/month or $456/year

    • Perio Complete: $58/month or $696/year

🎁 Membership includes:

    • 2 Professional Cleanings
    • 2 Regular Exams
    • Routine X-rays
    • 1 Emergency Exam
    • 10-30% Savings on additional procedures
    • Child Complete Plan also includes 1 Fluoride Treatment
    • Perio Complete Plan includes 3 Perio Maintenance Visits instead of 2 cleanings

3. Do you see kids?

  • No, we do not see pediatric patients
  • Our services are available only for individuals 18 years and older

4. Where are you located? What are your hours of operation?

13. Do you accept walk-ins?

  • No, we do not accept walk-ins
  • Please call 518-308-4200 to schedule an appointment

 

🔍 Preparing for Your Visit

5. What should I do before my new patient exam and cleaning (first visit)?

  • Complete these steps before your first appointment:
    • Sign into the patient portal link (provided via email or text)
    • Complete your documents (medical history form, privacy policy, and HIPAA form)
    • Bring a list of ALL medications you are currently taking
    • If you have asthma, bring your inhaler to ALL appointments
    • Bring your insurance card and Driver's License

6. Should I premedicate before my appointment?

  • If you have more than one artificial joint and a compromised medical history, premedicate before ALL appointments (excluding consultations and non-invasive interviews)
  • We generally require medical clearance from your doctor before starting a treatment plan for patients with artificial joints

7. I have a complex medical history; will I need medical clearance?

  • If you have a complex medical history (severe heart conditions, breathing disorders, history of surgeries, blood thinners, etc.), you will likely require medical clearance
  • ⚠️ Please complete your medical history form BEFORE the appointment so we can determine if medical consult will be required

8. I have extreme dental fear. How can I relax during my appointment?

  • 🎧 Bring earphones to listen to your favorite music
  • We provide stress balls to aid in stress relief
  • On your first visit, we will discuss your dental anxiety and create a personalized plan to help manage it

12. I have old x-rays and records from my previous dentist. Should I bring them to my appointment?

  • ✅ Yes, please bring all old X-rays and dental records
  • These will be scanned into your patient profile for future reference

 

🔬 Services & Procedures

9. Do you extract 3rd molars?

  • We do not extract third molars unless they have severe mobility
  • Generally, we refer third molar extractions to an Oral Surgeon

10. Do you place implants?

  • Currently, we do not place the implant (screw)
  • We do restore the crown portion of the implant
  • We need your implant information (from your oral surgeon) before restoring with a crown
  • We can refer you for implant placement and then restore the crown in our office
